you will require continued care, I suggest you arrange to place yourself
under the care of another physician.
To help you to this end, I
have listed
several suggestions here. Please
do not use my name as your physician for such things as laboratory tests
or x-rays starting May 15th. I
will not be equipped to handle or interpret any report that might be
sent to me. I shall make your medical records available to you. As these records are confidential, I will require your written authorization to make them available. A form is included here for this purpose. Information
about my new address and contact numbers
can be found here. This
webpage and links will be here for
the next 6 months. I
am very sorry that I cannot continue as your physician.
