Dear Patients:  

As you were notified by mail in May of this year, I have closed my practice as of August 15^th, 2008.  

Please be advised that I can no longer renew prescriptions, complete forms or make referrals.

I will keep this page up for another 6 months for those who need access to the information previously provided both here and by mail.    

For those who require resources to find another physician, I have listed several suggestions here.  

Those who require medical records will find the legally required authorization form here.  As these records are now in storage, each request will require a fee of $75 to cover retrieval, copy and postage.  Requests sent without this fee cannot be processed.
